Transplants scare the hell out of me, if anything can go wrong with the plant it will be there. My only hope is that I have a good rootball that keeps roots and soil intact. Do you score your roots at all, or leave them be and replant?

I 100% agree with this ^Dont worry about transplants in veg brother. I can't tell you how many times I have watched landscapers get Midevil on new plantings and rarely have to replace a plant
I like to take a large McD's coffee cup poke some holes in the bottom . soak the soil plant the seed and let it grow in there till the roots come out the bottom and she has 4 sets of real leaves. I know its ready for its new home then. Rip the cup off and place the plant in her final home Haven't killed one yet doing that. so just leave the roots alone

when transplanting its best to let the pot dry out so the roots are holding the soil together, then turn the pot upside down put keep the stem of the plant between 2fingers.
so when you got the pot the right way up, slide your hand over the top of the pot so the stem of the plant goes between 2 fingers so your hand is completely over the top of the pot with the plant in, then turn the pot upside down and either give it a few taps with the other hand or give it a gentle squeeze all the way around the pot to release the roots from the edge of the pot, then the plant and root ball will fall into your hand, then pull the pot off and place the whole lot into a pre dug hole where your transplanting it to.
i find if its got roots growing out the bottom of the pot then these will get damaged when you try to take plant out the pot, but turning the pot upside down is the best method, never try pulling the plant out its pot by the stem, this will damage many roots, its best to let the pot dry out and turn it upside down with the stem between your fingers, thats the best method and causes less stress,
if you try doing it with wet soil then the soil will break up in your hand and you will have problems transplanting then and lots of transplant shock, if done right the plant wont suffer any shock at all, just water well after transplanting then job done
