This is how I do it.

I use a DSLR (an inexpensive entry level camera) set manual f20 fastest shutter (my cheap camera is only 200) that will work with flash

external flash (speedlight SB-600)

70-300 with VR lens (slow so it is also inexpensive and lighter) with auto-focus turned off.

a set of cheap extension tubes

With the extension tubes and manual focus you can rough focus using zoom on the lens (the long lens allows distance for the flash) Then I fine focus weaving back and forth till what I want is in focus and snap the shutter. All my pics are hand held.
 
see you got a DSLR!!! most of us poor souls use crappy point and clicks or even worse camera phones.

I just looked ion the interweb and the camera I have can now be had for about the cost of a fancy phone :cheesygrinsmiley: If you have a point and click or camera phone you can sometimes get even better images!! Why? because they give a greater depth of field to get more in focus. What do you use? Maybe I could help... maybe not but I will try.
