I've been using Cannabis for 10+ years for medical reasons. After having no help from any Dr to go the legal way I relied on the black market. The past year or so the quality has gone down and the danger has gone up. I tried going my local Dr route again with no success then I found this thread. I'll post my experience with the services I've tried so far.
Body stream medical replied the next day. They stressed that they can only treat a pre diagnosed condition and require documentation from a Dr.
The problem is my Dr will only write a note of diagnoses if it's being sent to one of his preferred specialists.
They charge $350 for one year service.
Medical marijuana services has not replied back to me yet. On their list of forms the instruction sheet that lists what documentation you need to provide to prove your diagnoses opens up blank. All the other forms open no problem. Maybe MMS has the ability to diagnose you whereas body stream cannot. The MMS site says they charge $150-500 for the first year. Maybe the higher price is for diagnosing.
Canadian medical services replied with an auto message saying they will contact shortly.

I'm in the same boat as you are, Jordan. I have been waiting 8 days (2 weekend days, in there) for a reply from MMS. This full week has gone by (Friday night 11:40pm) with nary a word. How long have you been waiting?

It's only been 3 days for me. I was hoping for a faster response. I'm not sure where to send my registration. I can't find and address anywhere on their site. I hope they reply soon. Let me know how it works out for you.

I have them all filled out and ready to go but was hoping to talk to someone before I send them. I wish I could mail them in, I just have a tablet and my phone so I can't scan them in. I'll have to hunt down a fax machine now. Did you send in the optional forms too? I think I might just incase they ask for them later. I want to keep the delays down.

Oh yea! I always forget about staples. The one right by my house just closed down so I'll be venturing to Oshawa. The "instruction read first" form opens blank for me. I was hoping the mailing address was on there lol. I really hope MMS works for me. I don't care how much they charge. I've been looking at med releaf as my LP if I get approved.

Avoid MMS people, they are going to charge you 400 and send you to bodystream anyway, Everything they do is SLOW, they also wont give you a receipt when they charge you. I wanted to claim it back on my taxes along with all of my orders ive made with tilray. Bodystream does 6 month scripts, but no charge after the first 6 months. I just had my renewal appointment via skype on Wednesday and i switched LP's from Tilray to Maricann Inc looking forward to making my first order with them, ill keep you guys posted.
Also, I havent put in an order with tilray since DEC 10.. Sorry but i cant afford $15/g + taxes lol its really a damn joke..So i have been ordering from Compassion Clubs and dispensaries, really great quality and no tax!

I received an email from body stream but they are insisting on documentation from my personal Dr in the form of consultation notes or a letter of diagnoses. He will only send those forms to a specialist he recommends. If I could get those forms body stream looks like the best avenue to take. My Dr says my conditions of back pain and sleep deviation is too easy to fake so he is stingy with referrals of any kind.

WOW looks like you need a new doctor, When i signed up with MMS all i sent was a copy of all my prescriptions for the last 3 years. I have crohns so it may be a little easier to get a script then if you have back pain. My Doctor wouldnt sign, but said if i need a letter of diagnoses he would write one for me and i can send to whoever needs it.

Canadian marihuana services called me back the other day. Very friendly on the phone and quite helpful. Again they can only help with a pre diagnosed condition and require a substantial amount of medical history to verify it. I'm still waiting for MMS to get back to me. They seem to be the slowest and most expensive but the easiest to get a script from.

I've found a new service that I'm going to try while I wait for MMS to get off their ass.

They charge $400 for a year and send you the hard copy of the script via express post.
They do require some documentation but it can be as simple as an ailment report or a simple Dr note saying you were seen by a Dr complaining of pain etc. you can also forward previous prescriptions you've been given for the condition. I started the online application and there is a spot to upload your documents. the difference between this and other services is they only give you the script. You still have to register with an LP and send the forms yourself. Ill keep you guys posted. Just getting sick of waiting for MMS.

I was led to believe MMS had their own DR's that could diagnose and prescribe me. What a waste of time, I could have gone to Toronto and walked into a clinic. This whole process really sucks...

Wow--sorry you had such a crappy experience. When I went through MMS they didn't ask for any info from my family Doctor or any medical history at all. I filled out the forms, paid the $350, and MMS just setup the Skype appointment with a Doctor for me.
