Hi Michelle, Just to clarify, the brass firehole insert does not fall out frequently under normal circumstances. It is permanently bonded to the stone. Fish had broken the bond on his inadvertently, and so it wasn't actually attached when he wrote that. We resolved his issue immediately and have not had any reports of problems with the insert from other customers.
