Where's the pathologist

Could someone try and shed some light on this ? I'd like to prevent it in the future, if possible.

Medium: Soil with NPK 9-5-3 organic fertilizer

Started under terrible lighting conditions for first 5 days.

We are talking about the seedling on the far right ..

What changed is that on day 10 I added a bit of NPK 12-10-18 universal garden fertilizer (as in a tablespoon filled with pellets) and then added more soil, to help support the stems that got too long due to my bad lighting conditions at start. Then I generously watered them till a bit of water started coming out of the pot.

Day 12: not looking that good .. yet the others do fine .. could it be overwatering ? I'll just leave it alone and see tomorow

Day 14: At this point I thought it must have been too much nutrients near the roots or something .. and decided to flush. I used more then 4 times the volume and then poked holes in the dirt to create more surface area for faster drying, and pot a fan on it (not shown here)

This is how Benjamin (the worst seed and smallest plant) is doing at day 14:

Day 15 .. notice the two bright green tips .. still hope ...

End of day 15 .. hope fading:

Given up, extracted .. wtf

Is this the root system one would expect at day 10 ?
Is this nute burn ? If so, why no signs of that on the others ? I used the same dosage of nutes on all of them...
Could this be a not so good seed ?

Any insights would be appreciated, thanks in advance.
